Here is the link to our digital intake paperwork in order to complete all the information that we need and to save your credit card on file:

We offer two options for evaluations:

1. $300 for the initial evaluation with no report,
2. Or an additional $500 for a detailed written report.

In both cases we will have a plan of care and goals, but you will have a written report with the second option.

• Sessions are $225 per 40 minute session.
• Sessions outside of our typical service area may incur additional travel fees.

No charges are ever billed until after therapy sessions are completed.
